The structure that passes review

Start from the risk assessment: every significant hazard it identifies must show up as a control inside a step, not in a separate list nobody reads
Open with the job facts: site address, client, scope in plain words, programme, who supervises and how operatives are briefed
Sequence of operations in the order the work actually happens: access, set-up, the task itself, inspection, housekeeping, close-down
Name the plant and equipment, with the certificates and competence that go with them: CSCS, PASMA, IPAF, face-fit, calibration where it applies
State PPE per task step rather than a generic list, and the emergency arrangements for the specific site: first aid, fire, rescue where relevant
Keep it job-specific: the site name in the body must match the cover, and every hazard row must earn its place for this task
