The checklist a CPP is judged against

Project description: what is being built, where, for whom, the programme and key dates
Management arrangements: who holds which CDM duty, how the work is supervised, how subcontractors are controlled and coordinated
Site rules: induction, signing in, PPE standards, permits, deliveries, exclusion zones and traffic management
Welfare: toilets, washing, rest and drying facilities meeting Schedule 2 from day one
Health and safety arrangements for the specific risks of this project, including any work involving particular risks
Fire and emergency procedures: alarm, escape, assembly, first aid and accident reporting arrangements
Arrangements for developing and reviewing the plan as the construction phase proceeds

Questions, answered

Is there an official construction phase plan form?

No single mandatory form exists. CDM 2015 requires the plan; HSE guidance L153 sets out the content it should cover. Any format works provided the content is there and reflects the actual project.

Can I reuse the plan from my last job?

The structure, yes. The content, no. A plan that still names the last site's address, programme and risks is exactly what gets bounced, because the regulation requires a plan for this project.

What gets a plan rejected most often?

Generic padding that clearly was not written for the job, missing welfare or emergency arrangements, and no real management arrangements: nobody named, no induction, no site rules. Those are the first things a reviewer looks for.
