When the regulations require one

Every construction project needs a CPP under CDM 2015 regulation 12, drawn up before the construction phase begins and developed as work proceeds
More than one contractor on the project: the principal contractor must draw up the plan
Only one contractor: that contractor carries the duty to ensure a plan is drawn up
Domestic jobs included: on projects for householders the client duties pass to the contractor or principal contractor, and the CPP duty stands
Notifiable projects (over 30 working days with 20 plus workers at once, or over 500 person-days) also require an F10 notification to the HSE
The plan must be proportionate: a loft conversion does not need a motorway-scheme document, but it does need a real plan

Does a small domestic job really need a construction phase plan?

Yes. CDM 2015 applies to all construction work, domestic projects included. For a small job the plan is short and proportionate, but it must exist and be in place before work starts on site.

Who is responsible for drawing the plan up?

Where more than one contractor is involved, the principal contractor. Where there is only one contractor, that contractor carries the duty. On domestic projects the client duties pass to the contractor or principal contractor automatically.

When does it have to be in place?

Before the construction phase begins. That is why the 48 hour clock matters: order before your start date, not on it.
